    Check them drive shaft u-joints.
Do you feel the clunk or just hear it?

i just hear the clunk noise if i let off the gas after it shifts to 2nd. but i even hear a rattle noise at certain speeds (25-40mph) also when i let off. my exhaust is new so nothing loose there. i tried moving the u joint with my hand to see any play but it was solid, but the truck was on the ground in park. not sure the correct procedure to check them. I'm thinking of Spicer u joints if i have to change them

A bad u-joint may not move by hand. If they are original and you have a lot of miles I'd recommend you replace them. Spicer u-joint for sure.
Also check engine and transmission mounts. It sounds like something related to the engine torquing and moving since you mention it happens when getting off the throttle.
