Have you checked the throttle position sensor?
You can probe the middle wire with a volt meter. With key on and engine off you should see a smooth sweep from around .7 volts to 3 volts as you open...
O2 sensors ever been changed?
Sadly no, not in the truck yet. I was just in the shop last night looking at it still sitting on the engine stand and was kicking myself in the ass. I need to make time to get it swapped soon.

I'd go with the 4 hole if you're replacing them. I doubt they make a difference but they sit far enough down in the injector...
