Finally finished installing my B&M Mega shifter in my 04 AWD QC. Got it to work w/ the 45rfe trans.
Here's a few pics of the install.

Needed a custom boot as the rubber boot was too thick. I think it looks similar to a Hammer shifter boot really.

Thanks to ADakotaRTGuy and his thread on the R/T Club site ( http://dakotart.com/forum/showthread.php?t=10726&highlight=shifter ) for all his help. Console and bracket for shifter are his design and where build by his welder and glass guy. Console mod and bracket can be ordered to do your own if you wish. I can answer any questions if anyone has any on getting this to work for the 45rfe using the stock cable mounting bracket and modified stock shift lever on the trans end.

The shifter is a non-truck version/ non-console version universal B&M Megashifter and I used the stock cable bracket (w/ 3 correctly sized washers), and lever on the trans end. Just had to modify the lever by a bit of grinding, cutting and drilling.
Here's a pic of the bracket from the post I referenced above:

Got the Steering column shroud (part # is 5GW81DX9AE,,, $28.35 for both pieces) and a replacement console section to cut into from my local dealer. I explained what I was doing to the parts guy and he gave me some price discounts. One thing I found about the lower shroud, the floor shifter version has a hole for a key lock mechanism (flip the switch to remove the keys) that you might want to cover/fill. I cut a piece of soft rubber (similar texture as the little padded area under the key hole) to use as a plug and put it in place once I determined it cleared the internal column parts as it (it's about 1/2" thick).

Painted the shroud black, to match my original, after spraying on some texture I bought from the local auto paint supply. I tried different colors, but could not match the color for the console. It did not match the paint code from the dealer and the local auto paint supply was not much help either. I figured if I could not match it perfectly it would look bad if I tried and missed on the color, so I went w/ black instead. Like the shroud.
